Art Exhibition Application Form

All art, prints, and/or craft work(s) must be created by the applicant—no reproductions of other people’s works. Use of trademarked or copyrighted materials is not permitted, unless the artist provides (and includes with each object) a statement from the original owner providing legal and written consent for its usage, or if the work is significantly altered from its original source material and considered Fair Use.

I certify that I am the original artist and owner of the submitted artworks above, and I confirm that my entries are not copied from any copyrighted materials without permission nor infringe upon any third-party intellectual property rights to the best of my knowledge.

Yes, I agree with the terms of the Exhibition Agreement. I also understand that I only need to provide a signed copy of the Exhibition Agreement once a calendar year.
